General assembly of Polish Society of Otolaryngologists, Head and Neck Surgeons Delegates will be organized on June 15, 2016 in the Philharmonic - Sala Kameralna  (Małopolska Street 48)

PARTICIPATION

Application of participation can be done by using the form available in the Registration section
 
The ones who would like to participate in the  Convention must fill and send back the participation form together with the proof of payment

Applications will be accepted until June 5, 2016. After that date, only if the limit of participants is not exceeded.
Participants of the Convention will receive the confirmation of the application acceptance by e-mail or post.

PARTICIPATION FEE

ATTENTION: the fee is determined by the date of payment, not the date of registration.

700,00 PLN ? application and payment from May 1 to June 14, 2016
900,00 PLN ? application and payment at the Convention (only if the limit of participants is not exceeded)

exempt from the participation fee

Accompanying person
200 PLN (entitle to participation only in the convention opening ceremony and in the accompanying event)

Cancellation of the ordered services, without financial consequences must be made before April 30, 2016.
After that date the cancellation fee (50% of the participation fee) will be charged.
The difference will be paid after the convention.

LUNCH

will be served on June 16 and 17 (Thursday-Friday) from 1pm to 2pm.
Lunch will be given on the basis of the lunch coupons that will be received during the registration at the Convention Office.

ID badges

Convention participants are required to wear the badges received during the registration.
